PLEASE NOTE: THE SPORTS PHYSICAL CLEARANCE PROCESS HAS CHANGED EFFECTIVE JULY 1, 2024. Please read all the information below carefully****
New Jersey State Law requires that all students that plan to participate in the high school athletics have a physical exam that is valid for 365 days and must be current on the day of the first practice.
The New Jersey Department of Education (NJDOE) has changed the sports physical form submission process. The sports physical examination forms no longer needs to be submitted to the student's school district. A PPE MEDICAL ELIGIBILITY FORM will be REQUIRED. The PPE Medical Eligibiltiy Form should be in the original form.
If the physical exam was completed more than 90 days prior to the first practice, the parent must also complete a Health History Update Questionnaire.
If the school doctor completes the physical exam AT THE SCHOOL the Health History Form needs to be filled out and signed by a parent/guardian prior to the exam AND the Annual Athletic Pre-Participation Physical Examination Form. Also, any student who has had a recent injury or illness must bring a doctor's note for clearance, NO EXCEPTIONS.
If the student's Healthcare Provider completes the physical exam, the Health History Form and the Pre-Participation Physical Examination form are KEPT by the healthcare provider. The PPE Medical Eligibility Form must be completed by the healthcare provider and submitted to the school district. This is the only form that is REQUIRED to be submitted to the nurse's office.

As of July 1, 2024, per NJ Administrative Code N.J.A.C. 6A:16-2.4, 2.2(h)1iv, physicals need to be conducted at the student’s medical home. We are no longer able to offer physicals at the school for all students. If you do not have a primary medical home, are without medical insurance, or have special circumstances, please reach out to the nurse’s office to assist in making arrangements for your athlete to receive a sports physical.
