You will see information and links for physical paperwork, concussion and some other forms that are necessary

2. Please scroll to the bottom of the page where the link for Activity registration is located-this is where athletes should register

3. Only Fall sports will be listed, we will open for Winter sports in mid October and Spring sports in January

PLEASE NOTE:  THE SPORTS PHYSICAL CLEARANCE PROCESS HAS CHANGED EFFECTIVE JULY 1, 2024.  Please read all the information below carefully****

REQUIRED sports forms for the nurse:

1. Updated Preparticipation Physical Evaluation Form  and History Form 

Physicals are valid for 365 days. The Athletic Pre-Participation Physical Exam and History Form will remain with your child’s physician and is no longer submitted to the school nurse. 

2. Preparticipation Physical Evaluation Medical Eligibility Form

This form will also be completed by your child’s physician and is required to be submitted to the school nurse.

3. Health History Update Form 

This form will be required if your child’s physical was completed over 90 days prior to the start of the season.

As of July 1, 2024, per NJ Administrative Code N.J.A.C. 6A:16-2.4, 2.2(h)1iv, physicals need to be conducted at the student’s medical home. We are no longer able to offer physicals at the school for all students. If you do not have a primary medical home, are without medical insurance, or have special circumstances, please reach out to the nurse’s office to assist in making arrangements for your athlete to receive a sports physical.
